Skilled Nursing Facilities

Our therapy teams work seamlessly with our SNF partners to provide therapy to patients in need of either short or long-term care. We know that recovery is deeply personal, and we strive to fully understand the rehabilitation goals of each patient and their loved ones. We consistently draw on our breadth and depth of industry knowledge to provide optimal patient care, which is supported and consistently improved by tracking and measuring our patient outcomes.

Synergy’s skilled nursing care begins with an on-site patient assessment and continues through discharge planning. During a patient’s stay our fully-licensed therapists deliver high-quality clinical care by implementing proven clinical protocols supplemented by the compassionate care that each patient deserves. In addition, we implement clinical programs aimed at reducing rehospitalizations to facilities whose patient population can benefit.

We work with our partner facilities to manage rehabilitation programs for many of the most common diseases and conditions, including:

  • Acute medical conditions
  • Fractures or joint replacements
  • Interim medical care following a hospital stay
  • Respiratory deficiencies
  • Parkinson’s disease
  • Terminal illness care
  • Stroke recovery
  • General rehabilitation due to weakness or physical decline

Independent and Assisted Living Facilities

Independent and assisted living providers that proactively anticipate and address the changing needs of their residents are able to significantly reduce the need for resident relocation. Outpatient rehabilitation services can be an integral part of empowering residents to age in place.

Some residents may conclude home health therapy and continue to benefit from additional therapy services in order to achieve the highest practical level of functional living. It is also common for residents to be referred directly to outpatient therapy for skilled rehabilitation services. We work with your residents and their physicians to determine the best outpatient speech/language, occupational or physical therapies to return the resident to his or her maximum potential.
